1. Fire Rated Door not latching.
The Findings Include:
On tour October 25, 2010, with Maintenance, a (1) hour Fire rated Corridor door (Hallway by Ward Entrance), Unit 31, did not latch at top and bottom, they are required to have (2) points of latching. This condition could result in (2) Smoke Compartments to become involved in a Fire/Smoke situation. According to LSC 101, Chapter 19, 19.3.6.3.11, "Dutch doors shall be permitted where they conform to 19.36.3 and meet the following criteria: (1) Both the upper leaf and lower leaf are equipped with a latching device. "
2. Based on direct observation, staff interview and document review, the facility did not meet the requirements for frame and door rating labels to maintain their fire resistance rating, resist the passage of smoke and provide for a safe environment.
The findings include:
While on tour of the facility October 25, 2010, accompanied by the Director of Maintenance, it was observed that frame and door rating labels were painted over throughout the facility. This condition could indicate a weakness in the fire prevention system.
