Facility failed to maintain door in double egress door accessible in accordance with the 2000 edition of the Life Safety Code NFPA (NAtional Fire Protection Association) 101. Findings included the following:
Surveyor observed between 2:00 and 2:30pm on 4/03/14 that one of two cross corridor double doors to patient corridor from lobby were not unlactching readily.

Surveyor observed between 2:00 and 2:30 pm on 4/03/14 that 2 20# nitrous oxide cylinder were not properly fastened in the medical gas storage room.

Facilty failed to maintain electrical panel board labels in accordance with the 1999 edition of the Standard for Health Care, NFPA 99. Findings included the following:
Surveyor observed between 2:00 and 2:30 pm on 4/03/14 that essential electrical system panelboards were not labeled completely. The emergency panels shall be distinctively marked so that they are readily identifiable as a component of the essential electrical system - NFPA 70, 1996: 110-22 and 700-9(a). They shall be labeled "LIFE SAFETY", "CRITICAL", or "EQUIPMENT" as applicable.
