PATIENT RIGHTS: CARE IN SAFE SETTING

Based on observation, interview, and record review, the facility failed to ensure that 2 of 2 staff members (Staff J and Staff K) providing direct patient care were certified in Basic Life Support CPR (Cardiopulmonary Resuscitation). Siince CPR is an emergency procedure that combines chest compressions with artificial ventilation as an effort to manually preserve intact brain function until further measures are taken to restore spontaneous blood circulation and breathing in a person who is in cardiac arrest, valuable time can be lost if the care giver is unable to perform this procedure.

Findings included:

Observation on 1/9/2020 at 11:00am showed Staff J in the bedroom of Patient #1. He was in the room alone with the patient.

In an interview with Staff J on 1/9/2020 at 11:00am, he stated he was providing a one-on-one with Patient #1. He also stated he was classified as a "sitter." He concluded by stating that he did not let the patient out of his sight and remained arms length from Patient #1, adding that his may mean taking the patient onto the outside play ground.

Record review of the personnel file of Staff J and Staff K showed that they were not certified in Basic Life Support CPR. The job description for a "Patient Sitter," approved 5/2018, showed the following position summary: "Under daily supervision of the Registered Nurse/Charge Nurse and directly to the Nurse Manager, the Patient Sitter observes the patient for any unusual behavior and assists in the prevention of any harm to the patient." Job Specific Responsibilities included: "... Exchanges and communicates patient care activities and meets all patient needs within shift parameters ..." Position Qualifications: Certification - none required.

In an interview with Staff A, B, and D on 1/9/2020 at 3:00pm, they all stated that the Patient Sitters should be certified in CPR. Staff D further stated she had just added "CPR Certification" to the Position Qualifications for a Patient Sitter.
