Based on visual observation the facility failed to maintain the separation of hazardous areas from other parts of the building, including the egress corridor. Hazardous areas are required to be constructed to resist the passage of smoke. The deficient practice had the potential to affect 18 of 18 residents.
Finding:
During tour of facility on 1/16/2013 observation revealed unsealed ceiling penetration in both boiler rooms, They consist of unsealed metal pipe and open holes.

During tour of facility on 1/16/2013 observation revealed no exit sign at cross corridor doors on the new wing and these doors are kept closed .
