Based on observation and staff interview, it was determined the hospital did not abide by their preventive maintenance program. (specifically as it applies to maintenance of the facility elevators.)
Findings include:
1. During a facility tour on 07/25/16 at approximately 11:15 a.m. it was determined that elevator "J" was out of service and the maintenance director was not aware this elevator was out of service and there was no signage indicating the elevator was out of service.
2. During a facility tour on 07/25/16 at approximately 11:30 a.m. it was determined that elevator "D" was out of service and there was no signage indicating the elevator was out of service.
3. During a facility tour on 07/26/26 at approximately 11:20 a.m. it was determined that elevator "H" call lights did not work.
4. During a facility tour on 07/26/26 at approximately 11:30 a.m. it was determined that elevator "R" call light face plate had a hole in it.
5. The aforementioned deficiencies were discussed with the maintenance director on 07/26/16 at approximately 12:00 p.m. and it was agreed these needed corrected.
