Based on tests to doors and observations made during the survey for life safety from fire performed on 10/22/15 from 8:30 am thru 4:00 pm with the physical plant engineer employee (employee #1) and safety officer (employee #2), it was determined that the facility failed to main entrance door do not close completely (do not latch) as required by the 2000 edition of the Life Safety Code of the NFPA Section 19.3.6.3.
Findings include:
1. Patient's sleeping room doors are not kept closed at all times on the second floor.
2. Self-closing devices shall be inspected because some of them are not working properly at rooms #201, #217, #218, #222, #223, #318, #322, #323 and #324.
