Complaint TX00316296 was conducted on site on 09/05/2019.

Finding include the hospital failed 1 of 1 Patient (Patient #1) the right to receive care in a safe setting, based off of record review and interview.

The hospital medical records reflected that Patient #1 had two different request for Physical Therapy. Patient #1's medical record revealed that he had multiple falls, and the family requested that he receive weekly physical therapy and he was independently walking with the assistance of a walker prior to his admission to the hospital. As he was receiving it 3 times per week at his previous placement.

The hospital medical records indicated that Patient #1 did have a Physical Therapy Assessment on 04/09/2019, after the second request was made. The findings of the assessment suggested that "will benefit from 24 hour/day supervision and assistance, as well as PT to help him become more independent."

Through interview with hospital staff #3, reported that they did not follow up to ensure the patient had recieved any physical therapy, prior to fall that occurred on 04/11/2019. Through interview with staff #3, reported that the hospital does not offer PT to patients. The PT would have to come through an outside source.

Through interview with hospital staff #2, it was reported to the surveyor that there is no physical therapy being offered to the geri patients.

Through interview with hospital staff #5, it was reported to the surveyor that the psyhiatric hospital does not offer physical therapy to those patients in need of that service. It would have to be contracted out. Through interview with hospital staff #5, it was explained that the services were completed in the assessment, but there was no follow through on the doctors behalf to follow up on the assessment and ensure that the physical therapy services were provided to the patient.
