On 7-11-2016 During our regular inspection with Director of Maintenance and Surveyors 16776 / 34673 / 37694

1) The building overhang at the MRI Trailer is showing a add on to the overhang, the construction ( nonconforming construction Type) will have to be certified that the add on is the same construction rating as the main building.

2) On 7-11-2016 During regular inspection with Director of Maintenance and Surveyors 16776 , 34673 and 37694, found that fire proofing missing off and or scraped off the structural steel beams to the ceiling.

On 7-11-2016 During our regular inspection with Director of Maintenance and Surveyors 16776 / 34673/ 37694 found that Smoke doors in the Corridors had more than 1/2" gap space when the doors are closed. All of the doors need to resist the passage of smoke. Doors need to repaired or adjusted as per Code.

On 7-11-2016 During our regular inspection with Maintenance Director and Surveyors 16776 / 34673 / 37694. In the Kitchen area, Storage room Door was blocked open with a food cart. Note the door has a hold open device tied into the fire alarm system. Door can not be blocked.

On 7-11-2016 During our regular inspection with Director of Maintenance and Surveyors 16776 / 34673 / 37694. It was found that a Patio door out from the nursery area to a small enclosure was left unlocked, Note this is not and Exit door only a patio area. If any body had gone out side they could not get back to the building, a short wall built around this would have prevented anybody from making it to a public way. The Door locking system would have to be changed to prevent any body from being trapped on the outside.


2. Exit Door out of E.D. and a limited use door for this area had been " key locked". The only key is with the on duty charge nurse at the main nursing station ( and security or some one in maintenance. ) This was done for security reasons. This would have to be changed so that all personal at all time will be able to Exit the building.

On 7-11-2016 During our regular inspection with Director of Maintenance and Surveyors 16776 / 34673 / 37694 The MRI Trailer has been enclosed with a rubber type wall and ceiling for weather protection. The encloser is now attached to the building wall to allow weather protection at the building exit door to the trailer for the paitaints.

With the enclosed area inquestion, added sprinkler coverage will be needed to cover the 3 sides of the enclosed area. The building overhang has a sprinkler head for coverage to the overhang, this section will have to be checked to see that the coverage meets the Code.
