Bringing transparency to federal inspections

Based on a review of facility policies, tour of facility and staff interviews (EMP), it was determined that the facility failed to ensure and maintain a safe and sanitary environment to minimize all health hazards in the hospital for the protection of patients and employees.
Findings include:
1) Review of facility policy "Cleaning of Non-Critical Patient Care Equipment" reviewed December 1, 2014, stated, "... It is the policy of UPMC to clean and disinfectant shared non-critical patient care equipment in a manner that prevents the transmission of microorganisms while maintaining the integrity of the equipment..."
2) During a tour of 3 East Nursing Unit on October 1, 2015, at 11:00 AM, a basin containing blood stained gauze and debris from IV insertion was found on the cart in the clean utility room. During the tour EMP1 and EMP6 confirmed the above findings at the time of observation.
3) During a continued tour of 3 East Nursing Unit on October 1, 2015, at 11:20 AM, the inside bottom of the medication room refrigerator had a dark unidentifiable substance and debris. During the tour EMP1 and EMP2 confirmed the above findings at the time of observation.
4) A tour of the medication room on the 3 East Nursing Unit on October 1, 2015, at 11:25 AM, revealed an empty soap dispenser. Further observation noted no container in the dispenser to hold soap. Additionally, there was no alcohol based hand cleaner in the room. During the tour EMP1 and EMP6 confirmed the above findings at the time of observation.
