Based on clinical record review and interview, it was determined the facility failed to assure five of five (#5, #7, #8-#10) Medicare beneficiaries were provided "An Important Message from Medicare" within two days of admission. The failure to receive the standardized notice, "Important Message from Medicare", did not assure patients and/or their Representatives had the information necessary to make informed decisions regarding their rights as a Medicare recipient. The failed practice had the potential to affect all Medicare recipients who received care at the facility. Findings follow:
A. Review of clinical records on 07/26/12 at 1100 revealed five of five (#5, #7-10) Medicare Beneficiaries clinical records did not contained acknowledgement of the standardized notice, "An Important Message from Medicare".
B. Interview with the Director of Nursing on 07/26/11 at 1230 the above findings were confirmed.
