Findings:
The inspector observed, while accompanied by the Chief Operating Officer, Director of Plant Operations, and Director of Performance Improvement, during the hours of the inspection from 1:30 PM to 5:15 PM on 01/18/17, that there were the following issues:
1) The permanent generator was out of service and facility was using a portable generator. Please describe sequence of events in chronological order, when permanent generator was taken out of service, when facility started using portable generator, and what actions are being taken to ensure the portable generator is used only as a temporary solution.
