Based on observation, the facility does not provide fire safety to patients and staff related to integrity of the fire wall at 1 of 2 sites. (Newfane site)

Findings include:

Observation on 9/29/11 at 11:00 AM during tour of the Newfane site revealed four penetrations greater than 4 inches in the corridor wall above the ceiling between two doors to the laboratory.

This finding was verified with Staff #4 on 9/29/11

Based on observation, the facility does not provide fire safety to patients and staff related to safe path of egress at 1 of 2 sites. (Lockport site)

Findings include:

Observation on 9/26/11 at 11:00 AM during tour of the Lockport site revealed numerous boxes (30 or more) of medical records stored on the floor restricting egress travel from the medical records suite.

Observation on 9/26/11 at 11:20 AM during tour of the Lockport site revealed patient care equipment stored in the connecting corridor on 2 North Medical Surgical Unit restricting the path of egress to less than 44 inches. Open corridor access was 36 inches.

These findings were verified with Staff #7 on 9/26/11.

Findings include:

Observation on 9/26/11 at 2:00 PM during tour of the Lockport site revealed the odor of cigarette smoke in the corridor outside rooms #434 and 435. Interview with Staff #36 revealed it is the policy of the Reflections Addiction Recovery Center that no tobacco products are permitted. Staff #36 accompanied the surveyor into both rooms, and it was determined that a patient had been smoking in the restroom of room #434.

This finding was verified with Staff #4 and 36 on 9/26/11.
