The facility failed to ensure automatic fire sprinkler systems were installed in accordance with NFPA 13.

Observation determined:

1) Sprinklers in two (2) of two (2) "Pipe Chase Rooms" on the L-400 Wing were installed at a distance greater than twelve inches from the ceiling.

2) Sprinkler coverage in Treatment Room #427 was obstructed by a surface mounted light fixture.

The facility failed to ensure the automatic sprinkler system was continuously maintained in a reliable operating condition as required by NFPA 25, Standard for the Inspection, Testing and Maintenance of Water-based Fire Protection Systems.

Review of sprinkler records could not verify that the annual test of the automatic sprinkler system was documented in the past twelve (12) months.

The facility failed to ensure electrical wiring and electrical equipment met NFPA 70 requirements.

Observation determined multiple powerstrips and multiplug adapters located throughout the facility were used in place of permanent wiring.
