Observation determined:
1) Sprinklers in two (2) of two (2) "Pipe Chase Rooms" on the L-400 Wing were installed at a distance greater than twelve inches from the ceiling.
2) Sprinkler coverage in Treatment Room #427 was obstructed by a surface mounted light fixture.

Review of sprinkler records could not verify that the annual test of the automatic sprinkler system was documented in the past twelve (12) months.

Observation determined multiple powerstrips and multiplug adapters located throughout the facility were used in place of permanent wiring.
