Based on visual observation the facility failed to provide exit signage for all required exits. Exit signs provide a route for occupants to reach safety. The deficient practice had the potential to affect 36 of 36 residents.
Findings:
During the facility tour on 9/21/2017, between the hours of 8:30a-3:00p observation revealed exit signs located in the gym and the center of the Education building did not function properly when tested.
Interview with Maintenance revealed the facility was not aware that the exit signs were not illuminated.

Based on visual observation the facility failed to provide corridor doors that were not closing and latching in the frame. When the doors latch a smoke resistive seal is formed to protect the room ' s occupants. The deficient practice had the potential to affect 36 of 36 residents.
Findings:
During the facility tour on 9/21/2017, between the hours of 8:30a-3:00p observation revealed penetration in the door to blue room, short hall break room, B-10, B-11, B-13, B-16, B-18 and all water closets located throughout the facility. Observation also revelaed blue room door on day room side, door to B-5, B-11, B-10, Storage & B-17 would not latch in to frame.
Interview with Maintenance revealed the facility was not aware of the door to these rooms were not latching in the frame and that the doors did not create a seal, when closed, to prevent the transfer of smoke.
