Based on the review of abuse and neglect policies and procedures, personnel files and interviews with hospital staff, the hospital does not have mechanisms/methods in place that train employees what procedures to follow when a patient alleges abuse by a hospital employee. Nine of nine employee files reviewed did not contain evidence of employee abuse/neglect training. Interviews with hospital staff on 04/12/10 in the afternoon verified that the hospital does not have a training mechanism in place to train employees in the procedures to follow when they are a witness to abuse/neglect by a hospital employee or a patient alleges abuse/neglect by a hospital employee.

Based on review of hospital records and interviews with hospital staff, the hospital does not ensure that nursing care of each patient is assigned to nursing personnel who are qualified and competent to care for patients with specialized needs. Two ( F and J ) of nine ( A, B, C D, E, F, G, H, J) nursing personnel files reviewed of personnel taking care of Cardiovascular Intensive Care Unit (CVICU) patients did not have evidence of specialized training in the care of CVICU patients.
Findings:
1. On 4/12/2010 review of Employee F's file found there were no unit specific orientation and competency documents.
2. ON 4/12/2010 review of agency personnel files found there were no unit specific orientation and competency documents.
2. In a interview with CVICU manager April 12, 2010 at 1415, she confirmed there were no orientation or competency materials in Employee F's file. She stated she found the orientation and competency training materials in the employee's mailbox.
2. During the above interview, the CVICU manager stated agency staff utilized in the CVICU are oriented through the hospital's staffing office and do not complete unit specific orientation and unit specific competency training required of the hospital's staff. This was confirmed by Employee I, Staffing Coordinator with the hospital's staffing office in a conversation on April 12, 2010 at approximately 1430.
