Based on record review, policy review and staff interview, it was determined that the Registered Nurse failed to ensured the patient's monitor rhythm is assessed according to facility policy.


Findings include:



During review of the medical record for patient #1, it was noted that there was no documentation of monitor rhythm from 1:20 p.m. on 6/16/11 until 5:06 p.m. on 6/17/11. The facility ' s policy " Telemetry Monitoring and Documentation From Admission to Discontinuation " # 10=6-012, revised 5/11 requires that the monitor tech post an example of the patient ' s a.m. and p.m. rhythm. The policy also requires that the nurse is to review and initial the morning full disclosure rhythm document. During interview on 10/19/11 at 9:30 a.m., the Director of Quality confirmed the staff failed to follow the policy.
