Based on interview and record review the facility failed to provide an appropriate transfer to rule out an emergency medical condition for 1 of 1 patients (#1). Findings include:
On 8/19/10 at approximately 1030 an interview with the Vice President of Operations revealed that patient #1 was sent to a facility that did not have the capabilities to perform the needed diagnostic test to rule out an emergency medical condition. When the Vice President of Operations was queried about the incident she replied "There was a problem with communication between the nursing assistant and the physician. The physician thought the nursing assistant said that the receiving facility could perform the appropriate diagnostic test."
On 8/19/10 at approximately 1230 during review of patient #1's medical record revealed documentation in the the physician's progress note "Pt. is to go to Deckerville for ultrasound to rule out ectopic tonight. We called and they do have 24 hour ultrasound.... It is important you get the ultrasound at Deckerville tonight." The disposition for discharge was documented as home. The condition of the patient was not indicated.
