Based on review of documents and staff interviews, it was determined that the facility failed to provide an activities program directed by a qualified Activities Director.
Findings:
Review of policy entitled Swing Bed Admission, last revised 03/07, revealed that the resident was to be reassessed by all departments involved with his/her care, a nursing assessment was to be done every shift, and a daily recreational activities flow sheet was to be used to document interaction with the resident..
During the Entrance Conference on 4/2/2013 at 10:00 AM the facility's Chief Financial Officer stated there was no designated Activities Director.
During an interview on 4/3/2013 at 10:00 AM in the case managers' office, the RN Case Manager (employee #1) stated that there was no designated Activities Director and that the nurses and CNA's provided for patient activities. He/she added that the nurses and CNA's did not have the required training to be qualified to be an activities director. Employee #1 added that an activities assessment is done on admission and charting was done on the Recreational Activities Form.
