Base on a complaint investigation and nineteen (19) Physician Credential File (P.C.F.) evaluation with the medical director secretary employee #, 1 it was determined that the facility failed to ensure that all physician credential file was updated according facility medical by laws and to state local law 56 of 2001 and the Regulation of the Secretary of Health of Puerto Rico 9184 of July 1, 2020 for the construction, operation and licensing of hospitals in Puerto Rico to make this a mandatory requirement for 5 out of 19 P.C.F. reviewed. (P.C.F. #2, #8, #12, #15 and #19).

Findings include:

1. During the review of 19 physician credential file on 04/28/2021 at 9:30 AM till 10:30 am it was found the following:

a. Two out of nineteen physician credential file (P.C.F.) lack of an updated Professional Association (Colegio de medicos) (P.C.F #2 and #12).

b. One out of nineteen physician credential file (P.C.F.) lack of an updated Cardiorespiratory Certification (CPR) (P.C.F. #15).

c. Two out of nineteen physician credential File lack of an updated Hepatitis vaccine (P.C.F. #12 and #19).

d. Three out of nineteen physician credential file lack of an updated Influenza vaccine (P.C.F. #8, #12 and 15).

e. One out of nineteen physician credential file lack of an updated criminal record certification (antecedentes penales) (P.C.F. #12).
