Based on observation the facility failed to ensure that all corridor doors have positive latching mechanisms to keep the doors securely shut. The facility has in place roller latches in a corridor of staff offices. This affects all staff who work in that area. Findings included:
Observation on 3/11/10 of the office area near risk management revealed three doors that had roller latches on the doors to the corridor. The doors also have a deadbolt type lock which can be secured by a key but have no positive latching mechanism which will keep the door shut when closed without operation of the keyed dead bolt. It was noted on at least two of the roller latches that the roller part of the latch was missing and all that remained was the metal framework.
