The hospital reported a census of 102 patients. Based on observation, document review and staff interview the infection control officer failed to ensure staff followed infection prevention and control precautions for the three-compartment sink in the dietary kitchen.
Findings include:
- Staff E, observed on 8/19/13 at 2:10pm washing cooking utensils at the three-compartment sink in the dietary department. Staff E explained the first sink contained soap and water to clean the dishes, the second sink contained rinse water, and the third sink contained a sanitizing agent to sanitize the clean dishes. Upon request by the surveyor Staff E tested the sanitization water and the test reached a PH of less than 150ppm (parts per million) concentration.
Review of the manufactures recommendations for "Oasis 146" on 8/20/13 at 11:45am revealed the instructions failed to inform user how and when to test the sanitizer. The hospital's policy for Equipment and Utensil Cleaning and Sanitation reviewed on 8/21/13 at 8:30am directed, "...If using chemical sanitizing solutions follow the manufacturer's instructions ..."
Administrative Staff A, Administrative Staff B, and Administrative Staff C on 8/19/13 at 2:10pm acknowledged the hospital failed to assure sanitization of dishes washed in the three-compartment sink.
