This STANDARD is not met as evidenced by: Based on observations during the survey of the facility, on the afternoon of 10/02/2014, the required manual fire alarm pull boxes were not observed at two (2) facility exit doors: first, at the Materials/Kitchen exit door; and, second, at the 2nd Floor Stair #2 exit door. (NFPA 72 (2007 ed.), §5.13.6 - Manual fire alarm boxes shall be located with 1.5 m (5 ft) of the exit doorway opening at each exit on each floor.)

This STANDARD is not met as evidenced by: Based on observations of facility logs/records and during the survey of the facility, on the afternoon of 10/02/2014, the information indicating compliance with flame resistance requirements was not found on either the cubicle curtains in use nor in documents on file. (NFPA 101 (2003 ed.), §10.3.1 - Where required by the applicable provisions of this Code, draperies, curtains, and other similar loosely hanging furnishings and decorations shall be flame resistant as demonstrated by testing in accordance with NFPA 701, Standard Methods of Fire Tests for Flame Propagation of Textiles and Films.)
