Based on record review and interviews, it was determined the facility failed to enforce medical staff bylaws rules and regulations for 1 (#1) of 10 sampled records reviewed.

Findings included:

On 05/23/2016 at 1:00 p.m. a review of the medical staff rules and regulations, last updated and approved on 03/24/2015, stated: Routine consults, must be completed within 24 hours of the request.

A review of Patient #1's medical record revealed the patient was admitted through the emergency department on 04/15/2016 and discharged to home on 05/25/2016.
A detailed review of Patient #1's medical record revealed a request for an Orthopedic consult on 04/15/2016 at 11:20 p.m. and Interventional Radiology (IR) consult on 04/16/2016 at 11:10 a.m. and The physician progress notes document awaiting Orthopedic and Interventional Radiology (IR) consults.

The nursing notes on 04/19/2016 documented the patient has outstanding physician consult and the importance of completing consult and documenting consult in patient's medical record. The nurses notes on 04/22/2016 document the need for completion of an Orthopedic (Ortho) consultation, however, was informed Ortho "refuse to see patient on this admission and have no recommendations for his care. They will not address the consult at this time".

An interview with the Risk Manager and the Chief Executive Officer on 05/24/2016 at 11:30 a.m. confirmed the routine consult should have been completed within 24 hours of request.
