Based on observation and staff interview it was determined that the facility has failed to ensure that all required exits are installed and maintained in accordance with NFPA 101, 2000 edition.


On 09-27-11 at 1:32 PM, during the tour of this facility with staff member M, observation revealed that the required exit ramp from the A wing was not installed in accordance with 7.2.5 of NFPA 101. Also, the handrails were not in accordance with 7.2.2.4 of NFPA 101.

On 09-27-11 at 1:33 PM, staff member M stated during an interview that he was unaware that the ramp and handrails were not in compliance with code requirements. He also stated that he would schedule the needed corrections in a timely manner.

Based on observation and staff interview it was determined that the facility has failed to ensure that the fire alarm system is maintained in accordance with NFPA 72.


On 09-27-11 at 12:32 PM, during the tour of this facility with staff member M, observation revealed the following deficiencies with the facility's fire alarm system.

1. The fire alarm panel indicated a trouble signal.
2. A smoke detector is required in the room that houses the fire alarm panel.
3. Several abandoned fire alarm devices have not been removed.


On 09-27-11 at 12:47 PM, staff member M stated during an interview that a fire alarm company has been scheduled to repair or correct the trouble signal on the fire alarm panel. He also stated that he would have them add the required smoke detector and remove the old abandoned devices.

On 09-27-11 at 12:59 PM, during the tour of this facility with staff member M, observation revealed that the outgoing linen storage area was not provided with the required sprinkler coverage.

On 09-27-11 at 1:01 PM, staff member M stated during an interview that he was unaware that sprinkler coverage was required at this location. He stated that he would schedule the installation of the sprinkler coverage in a timely manner.
