Based on observation and interview the facility failed to maintain all sets of smoke barrier doors in good working order. This deficient practice affects all patients on that floor. The facility census for the main campus of SSM St. Mary's Richmond Heights was 256 and for the Cardinal Glennon (CG) campus was 138.

Findings included:

1. Observation during a tour of the St. Mary ' s facility conducted on 04/19/12 at 1:24 PM showed one of the two smoke barrier doors, on the Behavioral Health Unit adjacent to the east day room, would not completely close when released from the fire alarm system automatic hold open device.

2. Staff SSM-V, Team Leader Plant Operations, confirmed at that time one of the two smoke barrier doors would not completely close.

The National Fire Protection Association (NFPA 101) 2000 Existing Edition, section 19.3.7.6 states: Doors in smoke barriers shall comply with 8.3.4 and shall be self-closing or automatic-closing in accordance with 19.2.2.2.6.

The facility census for the main campus of SSM St. Mary's Richmond Heights was 256 and for the Cardinal Glennon (CG) campus was 138.

Findings included:

1. Observation during a tour of the Cardinal Glennon facility conducted on 04/17/12 at 3:50 PM showed the door to the linen chute discharge room in the basement area was propped open with a five gallon bucket.

2. Staff CG-N, Team Leader Plant Operations, confirmed at that time the door to the linen chute discharge room was propped open.

The National Fire Protection Association (NFPA 101)2000 Existing Edition, section 7.2.1.8.1 states: A door normally required to be kept closed shall not be secured in the open position at any time and shall be self-closing or automatic-closing in accordance with 7.2.1.8.2.

Based on observation and interview the facility failed to ensure all portable medical gas cylinders were individually secured. This deficient practice affects the operation of the facility. The facility census for the main campus of SSM St. Mary's Richmond Heights was 256 and for the Cardinal Glennon (CG) campus was 138.

Findings included:

1. Observation during a tour of the Cardinal Glennon facility conducted on 04/17/12 at 3:42 PM showed 14 Nitrous Oxide and 14 Nitrogen " H " size medical gas cylinders not individually secured in the medical gas room.

2. Staff CG-N, Team Leader Plant Operations, confirmed at that time all 28 " H " size portable gas cylinders were not individually secured.

The National Fire Protection Association (NFPA 99) 2002 Edition section 5.1.3.3.2 states: Locations for the central supply systems and the storage of medical gas shall meet the following requirements: (7) be provided with racks, chains or other fastenings to individually secure all cylinders, whether connected, unconnected, full, or empty from falling.
