Based on observation the facility failed to provide a properly sealed through the wall penetration for the electrical connection required from the rental generator to the existing transfer swicth located in the existing electrical room.
"Floor, wall and ceiling penetrations by pipes, ducts, and conduits or any direct openings shall be tightly sealed to minimize entry of dirt particles, rodents and insects. Joints of structural elements shall be similarly sealed." - HLR, 133.162 (d)(2)(B)(v).

Based on observation the facility failed to provide adequate clear path to the electrical room exterior door.
Wires from the temporary rental generator that connected to the existing transfer switch located inside the electrical room, inpeded a clear path to the entry of the electrical room. Wires were located on the ground and penetrated the exterior wall of the eletrical room by the hinge side of the electrical room door.
