Hazardous Areas - Enclosure

Based on observation and interview, the facility failed to maintain self-closing door for hazard areas. This has the potential of affecting approximately 1 patient, staff and visitors in one of 4 smoke zones, by allowing fire and smoke to spread from a hazardous area. This facility has a capacity of 10 and census of 1 patient at the time of the survey.

Findings are:

On 7/16/2019 at 3:17 P.M., observations revealed that the facility failed to provide a self-closing device that will completely close and latch the door in the frame for the Supply Room.

During interview conducted on 7/16/2019 at 3:17 P.M., Maintenance personnel stated that they were aware that the self-closing device did not completely close the door in the frame.

Fire Alarm System - Testing and Maintenance

Based on record review and interview, the facility failed to maintain the fire sprinkler system. This has the potential of affecting approximately 1 patient, staff and visitors in four of four smoke zones, by possible allowing the spread of fire and smoke throughout the facility. This facility has a capacity of 10 and census of 1 residents at the time of the survey.

Findings are:

On 7/16/2019 at 1:45 P.M., record review revealed that the facility failed to provide the required annual testing and maintenance of the fire sprinkler system.

During interview conducted on 7/16/2019 at 1:45 P.M., Maintenance personnel stated that they were aware that the date for the testing and maintenance for the automatic fire sprinkler system was out of date.

Sprinkler System - Maintenance and Testing

Based on record review and interview, the facility failed to maintain the fire alarm system. This has the potential of affecting approximately 31 residents, staff and visitors in four of four smoke zones, by possible not having an operational fire alarm system. This facility has a capacity of 10 and census of 1 residents at the time of the survey.

Findings are:

On 7/16/2019 at 1:30 P.M., record review revealed that the facility failed to have the fire alarm system inspected within the last six months.

During interview conducted on 7/16/2019 at 1:30 P.M., Maintenance personnel stated that they were aware that the fire alarm was due for the semiannual inspection.
