Based on observation, it was determined that the facility failed to ensure that a clear and unobstructed width of 4 feet for exit access was provided.
Findings include:
1. On 2/28/11 at 2:05 PM, in the presence of Staff #27, the exitway to Stair #5, in the SICU, was reduced to 24" wide, by four (4) stored beds that were not in use.

Based on observation, it was determined that the facility failed to ensure that the automatic sprinkler system was maintained.
Findings include:
1. On 2/28/11 at 10:30 AM, in the presence of Staff #27, three (3) boxes of supplies were stored within 18" of the sprinkler in the plumber shop.

Based on observation, it was determined that the facility failed to protect stored medical gas in accordance with NFPA 99.
Findings include:
1. On 2/28/11 at 11:55 AM, in the presence of Staff #27, fifteen (15) tanks of medical gas were unsecured in the A level tank cage.
