Based on observation and interview, it was determined the facility failed to maintain corridor openings in five locations, affecting one of one floor.

Findings include:

1. Observation on March 5, 2020, between 10:28 a.m. and 10:40 a.m., revealed the following:

a. 10:28 a.m., the patient room door 118 was not smoke-tight.
b. 10:35 a.m., the therapy door was held open by unapproved means (door chock).
c. 10:37 a.m., the distance between patient room 104 doors exceeded one-eighth-inch.
d. 10:38 a.m., the distance between patient room 102 doors exceeded one-eighth-inch, and the door lacked positive latching.
e. 10:40 a.m., the distance between patient room 101 doors exceeded one-eighth-inch.

Exit interview with the facilities manager on March 5, 2020, between 11:25 a.m. and 11:35 a.m., confirmed the corridor opening deficiencies.
