Based on observations made during review of the logs, policies and procedures of the facility between the hours of 10:30 am and 3:00 pm, while accompanied by two staff members representing the Maintenance Group the facility failed to provide logs documenting that the fire alarm system was tested monthly as required.

Based on observations made during the walk-through inspection of the facility between the hours of 10:30 am and 3:00 pm, while accompanied by two staff members representing the Maintenance Group the facility failed to meet the minimum requirements on the following issues:

1. In the MED GAS STORAGE area the freestanding cylinders were not all secured properly in accordance with the requirements of NFPA 99, 1999.

2. At the on-site Helipad, the "frequently" utilized helicopter landing pad had no lights that were in working order.


Additionally, based on observations made during review of the logs, policies and procedures of the facility between the hours of 10:30 am and 3:00 pm, while accompanied by two staff members representing the Maintenance Group the facility failed to provide logs documenting the annual testing on the effectivenees of the electrical power distribution and grounding systems.
