Based on observations, this facility is not assuring that smoke barriers are free of penetrations that compromise the fire-resistance rating of the walls and allow the passage of smoke and fire to another smoke zone. This deficient practice would affect all occupants in two of eight smoke zones. This facility has a capacity of 17 patients and a census of 8.

Observations on 10/6/10, revealed a gap (approximately 1 inch in size) around multiple cables and a large piece of loose drywall leaving a gap at the smoke barrier on 1st Floor West, across from room 103. Maintenance Staff A verified this observation.
