Based on tour and observations, 3 of 3 staff (A, C, D) interviews, the facility failed to maintain a sanitary physical environment to prevent cock roach infestation. This has the potential to affect all pts. in this 331 bed facility. The census for 5/8/13 was 69; therefore, this had the potential to affect all 69 pts. residing in the facility on 5/8/13. Cock roaches were found on 3 of 6 floors of the "St. Ann's building" which is attached to the inpatient hospital.

Findings include:

Upon entrance into the hospital at 8:25 p.m. on 5/8/13, Registration Clerk F was informed of the allegation and alerted on-call Administrator A via phone, then connected call to Surveyor at 8:35 p.m. When allegations of cock roaches being found in the "St. Ann's Building", A said, "We have occasionally seen cock roaches in that old building." A said would assign NOC (Nursing Operation Coordinator) B to assist with tour.

At 8:40 p.m. NOC B arrived to begin tour beginning on the first floor. Of the 7 rooms checked on the first floor, 3 of the rooms had cock roaches observed on the floor as follows:

Room Sa 1-09 had 1 non-moving cockroach on the carpet in the right corner. NOC B when shown said, "Ugh."
Room Sa 1-10 had 2 non-moving cock roaches on the floor. When shown to NOC B, B said, "oh".
Room Sa 1-02 had 14 non-moving cock roaches on the floor. When shown to NOC B, B said, "Oh yea."

At approximately 9:00 p.m. Environmental Housekeeping Staff C relieved NOC B so that B could give report to NOC D.

At 9:16 p.m. on the second floor in room Sa 2-16 2 cardboard sticky insect traps labeled "insect" were noted in the back of the room. On 1 of the traps there were 2 cock roaches stuck to the sticky surface. Housekeeping Staff C replied when seeing the cock roaches, "Oh yea. Do you want me to throw it (sticky trap containing the 2 cock roaches) out ?" Of the 21 rooms checked on the 2nd floor, 1 room had 2 cock roaches.

While touring the 5th floor with NOC D at 10:11 p.m. when D opened the door of room Sa 5-15 and the light was turned on, D said, "There's 1. It's a grand-daddy." One very large (approximately 2-2 1/2 inch long) cock roach was actively jumping around the floor on the left side of the room. In the storage room next to room Sa 5-13, 1 medium cock roach was found in the center of the room on its back. When pointed out to NOC D, D said, "Yea." On-call Administrator A relieved NOC D at 10:20 p.m. to continue the tour. Of the 21 rooms checked on the 5th floor, 2 of the rooms contained cock roaches.

When going through the automatic double doors to the sky-walk which connects to the inpatient hospital at 11:25 p.m., surveyor mentioned to Administrator A, that the laundry and house keeping carts are brought through these doors into the hospital making it possible for the cock roaches to get into the inpatient hospital areas, A said, "Yes it does."

At 11:30 p.m. when surveyor mentioned to Administrator A that perhaps the building may need to be sprayed for pests more frequently, A replied, "Yea that's what I was thinking."
