Based on record review and interview, facility staff failed to inform patients of their Medicare discharge rights at discharge for 3 of 5 Medicare-eligible inpatients (Patient's #1, 5, 10) in a total sample of 10 medical records reviewed.
Review of Patient #1 medical record revealed admission on 10/21/2019 and discharge on 10/28/2019. An Important Message from Medicare (IMM) signed and dated 10/23/2019 was in the medical record. There was not a second IMM prior to discharge.
Review of Patient # 5 medical record revealed admission on 8/8/2019 to observation status. The Medicare Outpatient Observation Notice dated 8/9 was in the medical record. The patient was changed to an in-patient status on 8/11/2019. There was no IMM in the medical record.
Review of Patient #10 medical record revealed admission on 10/24/2019 and discharge on 10/27/2019. There was no IMM in the medical record.
