Based on record review and interview, the hospital failed to administer drugs in accordance with the orders of the practicioner responsible for the patient's care for 1 of 10 patients (Patient #10), in that, Patient #10 did not recieve her 3 doses of her ordered medication (Dalfampridine 10 mg BID).
Findings Included
The physician's order for Patient #10 reflected, "07/08/2015...Dalfampridine 10 mg BID."
The Medication Administration Record for Patient #10 reflected, "07/09/2015...9:16 AM Dalfampridine held...20:52 (8:52 PM)...Dalfampridine held...not available...07/10/2015...14:06 (2:06 PM)...Dalfampridine held...20:20 (8:20 PM)...Dalfampridine given..."
During an interview and record review on 10/05/2015 ending at 12:30 PM, Personnel #1 was asked for the documentation regarding the Dalfampridine. Personnel #1 pulled up the Medication Administration Record for review. Personnel #1 showed the surveyor the Dalfampridine documentation which reflected the medication was "held" on 07/09/2015 at 9:16 AM and 8:52 PM and again on 07/10/2015 at 2:06 PM.
