Based on a review of six open medical records and five closed medical records, it was determined the hospital failed to provide the notification of rights, "Important Message from Medicare" (IMM), in two of eleven patient records reviewed.
Patient #7 (Pt# 7), a Medicare beneficiary, was admitted for a scheduled surgical procedure that resulted in a seven day stay. Pt# 7 required aftercare home health services and durable medical equipment that required educational instruction for safe use. No evidence was found in the medical record Pt# 7 was provided the secondary IMM prior to discharge home with complex home durable medical equipment and home health services. The hospital also failed to follow policy # DPP04-17 ("Medicare Beneficiary's Hospital Discharge Appeal Rights"), which states "the IM notice must also be provided to each beneficiary within two calendar days of the day of discharge".
Patient #8 (Pt# 8) was admitted for less than 48 hour period for a fall and related injury. Pt# 8 was not provided with the initial IMM upon admission. Additionally the hospital failed to follow policy #DPP04-17 (titled "Medicare Beneficiary's Hospital Discharge Appeal Rights") which states the IMM will be provided "within two (2) calendar days of admission". Pt# 8 was discharged to an acute rehabilitation facility.
