Based on observation and interview, it was determined that the facility failed to provide a safe and sanitary environment for its patients and staff.

Findings were:

"OSHA/Bloodborne Pathogen Regulations Policy #138-030-060" stated in part "The facility provides sufficient housekeeping and maintenance personnel to maintain the interior and exterior of the facility in a safe, clean, orderly, and attractive manner."

Facility policy entitled "Discharge Room Cleaning Procedure" stated in part "High dust, beginning at the entranceway and working around the room in a circle. High dust horizontal surfaces above shoulder height starting opposite the restroom."

Tour of the facility on 6/2/15 revealed the following:

· Chipped paint/laminate cabinets in the kitchen. This made thorough cleaning of the cabinets impossible.
· Also in the kitchen, dried food was found under a stove and dirt build up was noted in corners on the floor which indicated improper cleaning of the area.
· The dishwasher had a "leak" (according to the Registered Dietician) which resulted in silicone calking materials separating from the machine.
· Two of two patient rooms in the nursing wing had high horizontal dust. One "clean" room (ready for a new patient) had a used towel and a shampoo bottle in the shower area.
· In one of two patient rooms, a metal bed frame was noted to have rust spots.
· An isolation cart in the medical surgical patient wing was noted to have chipped paint. This chipped paint made thorough cleaning the cart unlikely.
· A drawer in the patient nutrition room housed a dirty washcloth and debris. This indicated improper cleaning of the area.
· Several fluorescent lighting fixtures throughout the hospital were found to have dead bugs inside.

In an interview with the Quality Improvement Manager on 6/2/15, the above deficits were acknowledged.
