Based on review of the Rehabilitation Unit Staffing Grid and interview, it was determined there was no evidence the facility based staffing on the needs of the patients' in that the facility utilized a number matrix grid which based staffing on total number of patients (census). Staffing based solely on a number system did not assure patient needs were taken into account in determining staffing needs. The findings follow:

A. Interview with the Rehabilitation Unit Supervisor on 08/16/12 at 0930 revealed staffing was based on a number matrix system based on census.
B. Review of the staffing grid revealed staffing was based on the census.
C. The Risk Manager confirmed the findings in an interview on 08/04/12 at 1000. The Nursing Director of Rehabilitation revealed there was no policy to address patient needs in staffing decisions.
