Based on clinical record review and interview it was determined that seven (#2, #3, #8, #11, #14, #15, and #16) of seven (#2, #3, #8, #11, #14, #15, and #16) patients who underwent operative procedures contained operative reports that did not contain the time of surgery. Failure to include the time of surgery did not allow knowledge of which surgical procedure was performed in what order in the event of multiple surgeries in one day. Findings follow:

A. Review of ten operative reports for seven (#2, #3, #8, #11, #14, #15, and #16) patients revealed the operative reports did not contain the time the surgical procedure was performed. (Patient #2 underwent four surgical procedures.)

B. The above findings were verified by the Director of Quality at 1020 on 08/31/12.
