Based on observation, measurement, and interview, the provider failed to maintain clear door widths of at least 32 inches in five of five smoke barriers in the patient wings. Findings include:

1. Observation and measurement at 10:00 a.m. on 5/3/11 revealed the cross-corridor smoke barrier doors in the patient wings were only 29 inches wide and did not provide a clear opening width of 32 inches. Interview with the facility manager at the time of the observation revealed those doors were from the original construction.
