Based on observation and interview, it was determined the facility failed to maintain sprinkler requirements in several instances on one of eight floors.
Findings Include:
Observation on August 9, 2011, revealed the following:
At 11:45 a.m. there was excessive dirt and debris on sprinkler heads throughout the 4th floor west wing.
Interview with the Engineering/Maintenance representative on August 10, 2011, @ 14:00 p.m. confirmed the condition with the sprinkler heads on the 4th floor.

Based on observation and interview, it was determined the facility failed to maintain HVAC requirements in one instance on one of eight floors.
Findings Include:
Observation on August 10, 2011, at 11:39 am revealed the following:
There were eggcrate ceiling tiles installed in the storage room behind the Memorial Conference Center creating a plenum. Items above the ceiling are not plenum rated.
Interview with the Engineering/Maintenance representative on August 10, 2011, @ 14:00 p.m. confirmed the condition in the storage room.

Based on observation and interview, it was determined the facility failed to maintain cylinder storage requirements in one instance on one of eight floors.
Findings Include:
Observation on August 10, 2011, @ 10:00 a.m. revealed the following:
There was an unsecured oxygen cylinder on top of some bins outside of the EMS office in the Emergency Room area.
Interview with the Engineering/Maintenance representative on August 10, 2011, @ 14:00 p.m. confirmed the condition with the unsecured cylinder in the E.R. Area.

Based on observation and interview, it was determined the facility failed to maintain electrical requirements in one instance on one of eight floors.
Findings Include:
Observation on August 09, 2011, @ 13:30 p.m. revealed there were beds and wheelchairs blocking the electrical panels in the 2 west penthouse.
Interview with the Engineering/Maintenance representative on August 10, 2011, @ 14:00 p.m. confirmed the condition with the storage on the 2nd floor.
