Based on observation and interview, it was observed in the electrical room, located next to the exercise room near the Recuperation Center, three of three electrical panels did not have a minimum of 36 inches (three feet) working space in front of the panels due to equipment stored in the electrical room. The failed practice had the potential to affect all patients, visitors, and staff because rapid access to the panels could not be gained in the event of an emergency in addition to creating a potential fire hazard. Findings follow:

A. While touring the facility on 03/13/18 at 1:30 PM, observation showed a cart and two chairs obstructing access to three of three electrical panels in the electrical room next to the Recuperation Ward Gym. These items prevented the panels from having a minimum 36 inches of working clearance as required.
B. While on the tour of 03/13/18, the Director of Plant Operations verified the panels were blocked.
