Based on policy review, medical record review, and interview, the facility did not ensure blood pressures for Patient #2 were monitored every five minutes per policy.

Findings Include:

Review of policy "University of Rochester Department of Anesthesiology and Perioperative Medicine (URDAPM) Anesthetic Care for Magnetic Resonance Imaging (MRI) and High-risk Imaging," last reviewed 04/14/21 revealed that MRI patients should be monitored in a manner consistent with ASA" Standards of Basic Anesthesia Monitoring." ASA Standards of Basic Anesthesia Monitoring indicate every patient receiving anesthesia shall have an arterial blood pressure and heart rate determined and monitored at least every 5 minutes.

Review on 06/26/23 of the Medication Procedural document dated 01/27/23 revealed at 11:35 AM, the Propofol infusion (general anesthesia) was started. At 12:16 PM, the Propofol infusion was stopped.

Review on 06/26/23 of the Intraprocedural Flowsheets dated 01/27/23 revealed no blood pressures are documented between 12:08 PM to 12:19 PM.

Interview on 06/28/23 at 03:30 PM with Staff (E), Associate Quality Officer, verified this finding.
