Based on observation and interview with the Materials Manager, the facility failed to provide inspection, maintenance and testing of the fire sprinkler system in accordance with NFPA 25 (1998 edition) 5.2.1.1.1. Failure to inspect, maintain, and test system components could result in the system failing to perform as designed.
Findings include:
During the facility tour with Materials Manager on 07/21/2016 starting at 11:45 am the following were observed:
1) Dirty sprinkler heads in Admission Office.
2) Dirty sprinkler heads in Charge Nurse Office.
3) Dirty sprinkler heads at Nursing Station.
4) Dirty sprinkler heads in Room 371
5) Dirty sprinkler heads in Environmental Services Closet.
6) Dirty sprinkler heads in Storage Room.
7) Dirty sprinkler heads in Pharmacy.
8) Dirty sprinkler heads in Nourishment Room.
These findings were verified by the Materials Manager at the times observed.
