Based on policy review and interview it was determined medication orders are not reviewed for appropriateness by a Pharmacist before the first dose is dispensed. This failed practice did not allow the Pharmacist to resolve issues and questions regarding medications before the patient received the first dose of medication. This failed practice had the potential to affect all patients in the Facility receiving medication.
A. Policy 16-45 Medication Management-5 Preparing and Dispensing F During hours in which the Pharmacy is closed, any medication orders will be verified by the Charge RN (Registered Nurse) and another nurse. The Pharmacist on call will be paged by the nursing staff for any questions needed. The Pharmacist will review all medications the next day of service.
B. An interview conducted on 05-04-2017 at 0845 with the Facility (named Pharmacist). The Pharmacist verified the Facility practice was what was as stated in A.
