Based on surveyor observation and staff interview, it was determined that the facility has failed to meet the construction type requirement for a Type I - 332 building for health care occupancy (any height).
Findings are as follows:
Surveyor observations on the morning of January 9, 2015 revealed the following deficiency:
The electrical closet across from Room# 029 in the sub-basement of the Zecchino (ZB) building lacked required fire stopping for several floor penetration openings surrounding vertical steel pipe chasers.
The floor penetrations openings cause the entire building to be rated as construction type III (200), an unprotected 12-story steel structure. A building of this type cannot be any height with an automatic sprinkler system.
During interview on 1/9/2015, the facilities management representative acknowledged the above observations were in need of repair.

CMS Regional Office Final Rule entitled Automatic Sprinkler Systems: This regulation requires all Long Term care and Hospital facilities to be equipped with a supervised automatic sprinkler system throughout the facility by August, 2013.
Surveyor observation during the Life Safety Code tour with the facilities management representative on 1/8/2015, revealed that the supply closet in the administrative offices located on the ground floor of the Hasbro Children (HC) building, lacked required sprinkler protection.
During interview on 1/8/2015, the facilities management representative acknowledged the above named supply closet lacked the required sprinkler protection.
