Based on observation and document review, the facility did not ensure that patients received care in a safe setting. Specifically, two corridor doors on the first level of the Children's Pavilion inpatient psychiatric facility were held open (with door holders) and posed a ligature risk for patients.
Findings include:
-- Per observation of the facility's Children's Pavilion inpatient psychiatric facility, on 5/5/2025 at 10:38 am and 10:40 am, two corridor doors were held open using door holders on the first-floor level of the facility, posing a ligature risk to patients.
-- Per interview of Staff B, Assistant Director of Operations, on 5/5/2025 at 10:20 am, the two corridor doors on the first-floor level were held open as staff had to use a physical key to open them, and it was more convenient for staff to have them held open until badge access is enabled for the doors.
-- During interview of Staff C, Deputy Director of Nursing for Inpatient Psychiatry, on 5/5/2025 at 1:20 pm, they acknowledged the above findings.
