Based on observation, it was determined the facility failed to ensure the smoke detectors were installed at least 3 feet from the air supply registers and air returns.
The findings included:
Observations during the initial tour on 8/28/12 beginning at 8:40 AM revealed the smoke detectors in the following locations were not installed at least 3 feet from the air supply registers:
a. main entrance corridor by the gift shop.
b. corridor by the conference room.
c. emergency department corridor ambulance staff room.
d. emergency department entrance.
e. emergency department waiting area outside of the "on call" office.
f. emergency department vending area.
g. corridor by patient room 2012.
h. corridor by patient room 2006.
i. corridor by patient room 3006.
j. corridor by patient room 3016.
Observations during the initial tour on 8/28/12 beginning at 9:51 AM revealed the smoke detectors in the following locations were not installed at least 3 feet from the air returns:
a. corridor by the 2nd floor respiratory supply room.
b. corridor by the 3rd floor respiratory supply room.
These findings were verified by the maintenance supervisor and acknowledged by the assistant administrator during the exit conference on 8/28/12.
