Based on observation and interview the hospital failed to ensure all equipment was maintained in a manner to ensure an acceptable level of safety and/or quality as evidenced by failing to ensure expired supplies were not available for patient use.
Findings:

In an observation on 07/10/2023 at 10:40 a.m. of the ED trauma room revealed supplies in the pediatric code cart with an expiration date of 06/30/2023. These supplies included 4 tracheal tube oral/nasal uncuffed supplies and 3 IV start kits.

In an interview on 07/10/2023 at 10:43 a.m. S1DED verified the 4 tracheal tube oral/nasal uncuffed supplies and 3 IV start kits was expired.

POSTING OF SIGNS

Based on observation and interview, the hospital failed to ensure signs were posted conspicuously in places likely to be noticed by all individuals entering the emergency department (ED) specifying the rights of individuals under section 1867 of the Emergency Medical Treatment and Active Labor Act (EMTALA), with respect to examination and treatment for emergency medical conditions and women in labor, and to post conspicuously information indicating whether or not the hospital participated in the Medicaid program under a State plan approved under Title XIX. The hospital did not have EMTALA signage posted in the ambulance entrance, elevators within the ED that lead to the ICU and Behavioral Health Unit, ED entry way from the main hospital waiting room, and ED entry way from a public hallway that leads to other hospital departments.

Findings:

On 07/10/2023 at 10:33 a.m., a tour of the emergency department with S1DED was conducted. The tour revealed there was no EMTALA signage in the following entryways to the emergency department:
" Ambulance Entrance
" Elevators within the ED that lead to ICU and Behavioral Health Unit
" ED entry way from the main hospital waiting room
" ED entry way from a public hallway that leads to other hospital departments.

In an interview on 07/10/2023 at 10:54 a.m. S1DED verified there was no EMTALA signage in the ambulance entrance to the emergency department and that there should have been.

In an interview on 07/10/2023 at 11:15 a.m. S1DED verified there was no EMTALA signage in three possible patient entry points which included elevators from the ICU and behavioral health unit, entry way from the main hospital waiting room, entry way from a public hallway that leads to other hospital departments and that there should have been.
