Based on observation the facility failed to ensure that all doors to areas identified as hazardous areas did not have doors which remained closed or have automatic closers in place to protect the area.
Findings included:
During tour of the North campus on 8/17/10 the door to the soiled utility room near the post partum unit was noted to have a self closer on the door. The door when opened and allowed to shut did not close completely and did not latch providing protection or separation to the area.
During tour of the North campus on 8/17/10 observation of the door to the main kitchen which opens to the main corridor was noted to have a self closer on the door. When opened the difference in air pressurization kept the door from closing automatically and the door did not latch and remained open approximately two inches.
