Based on observation and interview during complaint investigation #TN00035651, it was determined that the facility failed to enable responding personnel to identify the location of a fire quickly and accurately. (NFPA 72, 1999 edition 1-5.7.1.1.)

The findings include:

Interview and observation with facility director of engineering services, conducted on February 20, 2015 at 11:00 a.m., confirmed the smoke detector activation failed to annunciate and identify the proper location of said device, resulting in fire and facility personnel taking 52 minutes to locate the activation origin.

This finding was verified and acknowledged by Director of Engineering Services during the exit on March 4, 2015.
